COVID-19 TRAINING

Our staff has completed thorough training for new safety policy and procedures based on the latest CDC recommendations.

 

DAILY SYMPTOM SURVEY

Our staff will complete a screening for COVID-19 signs and symptoms prior to seeing patients every day.

 

SANITIZED CLINIC

The office space has been thoroughly sanitized and has frequent routine cleanings throughout the day and between patient visits.

PRIVATE ROOMS

Each patient will have a private treatment room for their appointment. The room will be sanitized before and after every appointment.

KEEPING YOU SAFE

Your physician and staff will wear a mask if the community transmission rate is in the High range and will perform hand hygiene frequently.  At the request of patients, we are happy to wear a mask during your visit.  Please let us know if you would like that.

REPORT ANY NEW SYMPTOMS

Please call our office 734 213-2996 if you experience any COVID-19 symptoms before or after your appointment.

Screening Questions

In the last 10 Days, have you been in contact with someone with a confirmed diagnosis of COVID-19?

 

Have you tested Positive for COVID-19 in the last 10 days?

In the LAST 24 Hours, have you begun experiencing any of the following symptoms:

-Temperature greater than 100.0 F

-Cough/Shortness of Breath

-Chest Tightness

-Body Aches

-Loss of Taste or Smell

-Nausea/Vomiting

SYMPTOM SURVEY

A Greeter will ask you screening questions for COVID-19 signs and symptoms (see below) when you arrive.

TEMPERATURE CHECK

A Greeter will meet you to have your temperature checked.

PRIVATE ROOMS

A Greeter will escort you to the treatment room or sanitized waiting area.

SOCIAL DISTANCING

We know at times in your appointment you will need to be in close contact with  your doctor, but when possible, please maintain a 6 foot distance.

PLEASE BRING YOUR OWN WATER

Unfortunately during this time we are unable to offer drinking water to people visiting the office. Please bring your own water bottle if desired.
